- DBA Paperwork or Business License
Organization
- Employer Identification Number (EIN) /Social Security Number (SSN)
- Copy of Authorized Signers Documentation- For EIN, copy of minutes authorizing signers. For SSN, personal account should be opened.
Partnership
- Valid ID and Social Security Card (Personal)
- Employer Identification Number (EIN)
- Social Security Number (SSN)
- Copy of Authorized Signers Documentation- Partnership Agreement; section naming specific partners
Corporation
- Valid ID and Social Security Card (Personal)
- Employer Identification Number (EIN)
- Copy of Authorized Signers Documentation - Corporate Resolution or Certificate of Incorporation
